To request a certified copy of your CME Credit Transcript, please pick one of the options below.

Online Transcript Request (Complimentary)

- Please allow 30-60 days from the date of the conference for credits to appear.  (Some exceptions apply.)

- If you do not already have a password, you will be prompted to complete one after filling out the necessary information.

- Please use your email address to log in.  If we do not have an email on file, you will be prompted to enter the last 4 digits of your social security number. (This is a one time only prompt for customers without an email on file.)

- Please fill out all the appropriate information to make sure your account is up to date.

- Please keep your email address and password in your files as this is how you will log-in for transcripts and to register for classes online.
